A Lebro Center for Well Being offers our patient forms online 

to be completed in the convenience of your own home or office 

before you even arrive to our office. Download the necessary form(s), print them out, and fill in the required information. Fax us your printed and completed form(s) to (207)439-9072 or you may email them to us at lebrocenter@gmail.com. You may also bring them with you to your appointment. Completing these forms prior to the day of your appointment allows us more time to focus on you.
